Make Your Child’s First Test an Eye Exam!
The Eye See... Eye Lean pilot program is currently limited to Hamilton Wentworth school boards. Families with children enrolled in kindergarten with the Hamilton Wetworth District School Board and Hamilton Wentworth Catholic District School Board can participate in Eye See…Eye Learn pilot program easily and and at no cost via the following steps:
Step 1:
Review the Eye See…Eye Learn information sent home by your child’s kindergarten teacher.
Step 2:
Call a local Eye See…Eye Learn optometrist to book your child’s appointment.
A list of participating Eye See...Eye Learn optometrists was enclosed with your Eye See…Eye Learn brochure or click here for a current list or call OAO directly at 1-800-540-3837.
Step 3:
Bring your child in for a comprehensive eye examination
Eye examinations are covered annually by OHIP for children 19 years of age and under. A comprehensive eye examination is painless and non-invasive and can identify vision problems including lazy or turned eye.
Please remember to bring your child’s OHIP card and the tear away voucher included in the Eye See..Eye Learn brochure that your child’s kindergarten teacher sent home with your child..
Step 3:
If necessary discuss a treatment plan with your optometrist
Children participating in the Eye See…Eye Learn program are entitled to a free pair of glasses (no cash value and only if corrective lenses are required)
Step 4:
Return the Teacher portion of the Eye Health and Vision Report (provided by the optometrist) to your child’s teacher. The report includes tips and recommendations to help your child in the classroom.
